PHIS provides a full range of Home care Services such as RN, LPN, CNA and CHM/HHA. Although we work directly with patients or primary caregivers to develop a program of services to patients. Simultaneously meeting the specific requirements and circumstances of the patient. We meet with the primary caregivers and patients at the patient’s home so that we can assess all of the medical, home care and social needs. Our team will develop care objectives and a specific program to meet the objectives and measure the results. We will interface with physicians and other medical professionals providing services to the patient, in accordance with all HIPAA requirements.
